Job Summary:

Kaiser Permanente is seeking a Licensed Practical Nurse (LPN) to join our team as a Clinical Health Worker (CHW) in our East Interstate Medical Offices. As a CHW, you will work closely with our healthcare team to provide high-quality patient care and support to our patients. This is a full-time, day shift position.

Responsibilities:
  • Provide culturally responsive care to Black patients, addressing racial health disparities and improving health outcomes.
  • Implement care delivery and social support models aimed at achieving health equity for the Black community.
  • Work with patients to develop and implement individualized care plans, focusing on health education and self-management.
  • Collaborate with healthcare teams to coordinate patient care, including scheduling appointments, ordering tests, and communicating with patients and families.
  • Provide emotional support and counseling to patients and families, addressing their physical, emotional, and social needs.
  • Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of solutions.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices in healthcare, applying this knowledge to improve patient outcomes.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current LPN license in Washington and Oregon.
  • Basic Life Support certification from the American Heart Association.
  • Two years of experience in a clinical setting, preferably in pediatrics, family medicine, or obstetrics and gynecology.
  • IV therapy experience or recent certification.
  • Familiarity with electronic medical records and computer systems.
Preferred Qualifications:
