Job Summary: As a Community Health Worker at Adventist Health, you will play a vital role in helping patients and families navigate community services and resources. You will advocate for individual and community health needs, providing direct services such as mammogram and diabetes screenings during community outreach activities. You will also support staff in helping patients adopt healthy behaviors and promote, maintain, and improve the health of community patients and their families.
Key Responsibilities:- Assist patients and families in accessing community services and resources
- Advocate for individual and community health needs
- Provide direct services such as mammogram and diabetes screenings
- Support staff in helping patients adopt healthy behaviors
- Promote, maintain, and improve the health of community patients and their families
- High School Education/GED or equivalent
- Associate's/Technical Degree or equivalent combination of education/related experience
- Successful completion of a Community Health Worker formal training program from an accredited college or other educational institution
