Job Summary

We are seeking a skilled Senior Community Health Worker to join our team at Providence Health & Services. As a key member of our primary care team, you will work closely with patients, families, and community members to provide culturally sensitive and patient-centered care.

Key Responsibilities
  • Build relationships with patients and families to understand their unique needs and preferences.
  • Provide education and support to patients and families on health-related topics, including disease management and prevention.
  • Connect patients and families with community resources and services to promote health and well-being.
  • Collaborate with healthcare providers to ensure seamless care coordination and communication.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements
  • Associate's degree in a related field, such as social work or health education.
  • 1 year of experience working in a community health setting, preferably with a focus on patient-centered care.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to navigate complex healthcare systems and connect patients with resources.
  • Basic computer skills and proficiency in electronic health records.
Preferred Qualifications
  • Bachelor's degree in a related field, such as social work or health education.
  • 3 years of experience working in a community health setting, preferably with a focus on patient-centered care.
  • Experience working with vulnerable populations, such as those with chronic illnesses or disabilities.
