Job Summary:

As a Licensed Practical Nurse (LPN) at the Center for Black Health & Wellness, you will play a vital role in delivering high-quality patient care and contributing to a positive care experience and work environment. You will work collaboratively with the healthcare team to provide culturally responsive care to Black patients, addressing racial health disparities and improving health outcomes.

Key Responsibilities:
  • Provide nursing and administrative support to clinicians and other healthcare team members.
  • Perform clinical tasks and procedures under the direction of a clinician, within the scope of your professional training and individual competency.
  • Contribute to the development, implementation, or modification of individualized patient plans of care under the direction of the RN/clinician.
  • Assist in coordinating patient care to support patients with complex health needs in navigating easily through the healthcare system.
Requirements:
  • Two years of experience with providing culturally responsive care to Black patients.
  • Two years of experience addressing racial health disparities and improving health outcomes of Black patients.
  • Two years of experience implementing or providing direct services/support through or in partnership with organizations that serve the Black community.
  • Graduate of an accredited school of nursing.
  • Practical Nurse License (Washington) within 6 months of hire or Compact License: Practical Nurse within 6 months of hire.
Preferred Qualifications:
  • Two years of nursing experience in assigned departments clinical area within an ambulatory care setting.
  • IV therapy experience or recent certification.
  • Familiarity in documenting in an electronic medical record system.
