About the Role

We are seeking a highly motivated and compassionate Senior Community Health Worker to join our team at Providence Medical Group. As a trusted member of our community, you will serve as a liaison between health/social services and the community, facilitating access to services and improving the quality and cultural competence of service delivery.

Key Responsibilities
  • Build individual and community capacity by increasing health knowledge and self-sufficiency through outreach, community education, informal counseling, social support, and advocacy.
  • Work collaboratively with the primary care team to effectively address the needs of patients in a patient-driven manner.
  • Develop and maintain relationships with community-based resources and advise patients on how to access these resources.
  • Provide support and guidance to patients, families, and caregivers to promote health and well-being.
  • Participate in quality improvement initiatives to enhance the delivery of care and services.
Requirements
  • Associate's Degree in an appropriate social service or health area.
  • 1 year of Community Health Worker experience or experience working with non-profits and local communities.
  • Ability to drive and provide proof of driver's license and auto insurance.
  • Completion of Community Health Worker Training and certification by the state of Oregon within 6 months of hire.
Preferred Qualifications
  • Bachelor's Degree in an appropriate social service or health area.
  • 3 years of Community Health Worker experience or experience working with non-profits and local communities.
