Hospice Traige LVN Position

Job Summary:
The Elizabeth Hospice is seeking a skilled Licensed Vocational Nurse (LVN) to join our team as a Hospice Traige LVN. In this role, you will provide professional nursing care to patients in their homes or facilities, working under the supervision of a Registered Nurse (RN).
Responsibilities:
- Assist RN in analyzing patient situations, anticipating problem areas, and taking appropriate action.
- Evaluate patient response to care, ensuring safe and timely outcomes.
- Perform licensed nurse skills, adhering to policies, procedures, and standards of care.
- Implement and document teaching plans, tailored to patient and family needs.
- Document all care provided, including telephone interactions with staff members.
- Enter or retrieve data from computer files, maintaining accurate patient records.
- Orient, instruct, and train assigned personnel.
- Perform phlebotomy as needed.
- Support agency in completion of assigned audits and office duties.
Requirements:
- Vocational Nurse Training.
- Minimum 12 months of experience as an LVN, with preferred experience in acute care, home health, or hospice.
- Current California LVN license.
- Familiarity with computerized bedside patient documentation.
- Ability to function as part of a team and take direction from other team members.
- Phlebotomy certification a plus.
- Bi-lingual (Spanish) a plus.
- Current California driver's license and proof of insurance required.
- Excellent communication skills with patients, families, and medical professionals.
- Ability to remain calm and flexible in stressful situations.
- Current CPR/BLS Certification required.
- Current TB test results required.
