The Elizabeth Hospice is seeking a skilled LVN to join our team as a Continuous Care LVN. This role requires on-site patient visits and provides professional nursing care under the supervision of an RN for assigned patients across the life span.
Responsibilities:- Assist RN in analyzing situations, anticipating problem areas, and taking appropriate courses of action.
- Evaluate patient response to care and make adjustments as needed.
- Perform licensed nurse skills, including phlebotomy, as stated in basic skills checklist and policies and procedures.
- Implement and document teaching plans based on patient and family needs.
- Document all care provided to patients, including telephone interactions with other staff members.
- Vocational Nurse Training.
- Minimum experience of 12 months as an LVN, with preferred experience in acute care settings, home health, or hospice.
- Current California LVN license.
- Familiarity with computerized bedside patient documentation.
- Ability to function as part of a team and take direction from other team members.
