Job Summary

We are seeking a skilled Licensed Practical/Vocational Nurse to join our team at Pacific Point Health Care Services. As a clinically owned healthcare agency, we prioritize providing exceptional care to our patients and their families.

Key Responsibilities
  • Provide direct patient care under the supervision of a registered nurse, following the plan of care and implementing current nursing practices.
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
  • Participate in coordination of hospice services, reporting identified needs to the interdisciplinary group.
  • Use equipment and supplies effectively and efficiently, providing appropriate pain/symptom management.
  • Participate in personal, professional growth and development, and quality assessment performance improvement teams and activities.
Requirements
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Three years nursing experience, with community health/hospice or medical/surgical experience preferred.
  • Currently licensed as an LPN/LVN in the State.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication, and organization skills.
  • Possesses and maintains current CPR certification.
