Job SummaryWe are looking for a dedicated and experienced LVN/LPN to provide high-quality patient care in a hospice setting. As a Hospice Nurse, you will work closely with our interdisciplinary team to deliver comprehensive care to our patients.
Responsibilities- Provide direct patient care, including administering medications, wound care, and other treatments as needed
- Conduct thorough assessments of patients' physical and emotional needs
- Develop and implement individualized care plans to meet patients' unique needs
- Collaborate with other healthcare professionals to ensure seamless care coordination
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making
- Graduated from an approved school of professional nursing
- One year experience as a Licensed Practical Nurse, preferred
- Licensed to practice as a practical nurse or vocational nurse in the state of agency operation
- Valid CPR certification
- Ability to travel to all business locations
