About the Role

We are seeking a skilled Hospice Nurse Practitioner to join our team at Curo Health Services. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and their families.

Key Responsibilities
  • Conduct Face-to-Face (F2F) encounters with patients to gather clinical information and support hospice diagnosis and prognosis.
  • Communicate findings to the hospice physician responsible for certifying or recertifying patient eligibility.
  • Perform physical assessments and provide clinical recommendations to the Interdisciplinary Group (IDG).
  • Notify the hospice agency when unable to complete a scheduled F2F visit.
Requirements
  • Master's or higher degree in Nursing.
  • Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
  • Current state license as a registered nurse.
  • Current state license or certificate as an advanced practice nurse.
  • Current national certification as a Nurse Practitioner (ANCC or AANP).
  • Collaborative agreement in place.
