Job Overview

Position Title: Housing Support Specialist

Employment Type: Non-Exempt

Role Summary: The Housing Support Specialist is responsible for ensuring the effective provision of high-quality services within housing initiatives.

Key Responsibilities:

  • Deliver field-based case management and assistance services.
  • Facilitate referrals for supportive services.
  • Maintain accurate documentation and up-to-date case notes in a computerized database system.
  • Process applications for housing.
  • Act as a mandated reporter for any suspected abuse or violation of rights.
  • Develop and execute personalized case management service plans.
  • Conduct thorough psychosocial re-assessments and update service plans regularly, at least every three months.
  • Assist clients in securing permanent housing, including providing location services and overcoming barriers to housing.
  • Build relationships with property owners and agencies offering affordable housing options.
  • Collaborate effectively as part of a multidisciplinary team.

Qualifications:

  • A minimum of 1 year of experience in a leadership capacity within social services or a related field.
  • A bachelor's degree in a social service-related discipline or an equivalent combination of education and experience.
  • Experience working with clients facing mental health challenges, chronic health issues, and substance use disorders.
  • Expertise in areas such as chronic homelessness, outreach strategies, intensive case management, mental health services, crisis intervention, and eviction prevention.
  • Proficiency in Microsoft Word, Microsoft Excel, and data systems.
  • Excellent verbal and written communication skills.
  • Strong organizational and interpersonal abilities.

Pre-employment Requirements:

  • Completion of a Tuberculosis Test.
  • Successful background screening.
  • Drug testing.
  • Verification of employment eligibility.
  • Willingness to obtain CPR/First Aid certification.
  • Possession of reliable transportation, a valid driver's license, proof of insurance, and ownership documentation.
