Job Description

Salary: $18 to $26

Location: San Rafael Office

Overview:

The Patient Intake Specialist is accountable for overseeing the entire patient intake process. This role involves collaborating with the intake team, fostering positive relationships with clients and referral sources, addressing client inquiries and concerns, and managing the verification and authorization of insurance.

Key Responsibilities:

  • Assess hospitalized patients referred for home care services.
  • Acquire a complete copy of the patient's medical history and physical examination.
  • Identify and confirm insurance coverage for home health care services.
  • Secure and document prior authorization for home care services from insurance providers.
  • Assist in obtaining supplemental managed care orders under the guidance of the HMO Intake Manager.
  • Negotiate payment rates for bundled home care services.
  • Work with hospital discharge planning staff, utilization review teams, and insurance case managers to ensure safe and timely discharges.
  • Consult with healthcare professionals to develop a coordinated home care plan.
  • Verify the completion of required face-to-face evaluations.
  • Engage with patients, families, and caregivers to discuss home situations, current needs, and relevant psychosocial factors.
  • Complete referral documentation that includes intake data, essential background information, hospital course, and care plans.
  • Maintain effective communication with hospital and insurance personnel, providing information on services and coverage issues.
  • Participate in patient care conferences and training sessions.
  • Collect and maintain statistical data on referrals and submit reports as required.
  • Attend meetings and participate in committees as necessary.
  • Notify the Director of Patient Care Services and Intake Supervisor of any delays in clinical care.
  • Perform additional duties as assigned by management.

Qualifications:

  • High school diploma required; background in secretarial work and computer skills preferred.
  • General knowledge of spelling, punctuation, grammar, medical records, and office procedures.
  • Familiarity with office equipment.
  • Basic telephone communication skills.
  • Proficiency in data entry and current software systems.
  • Ability to build and maintain effective working relationships.
  • Professional demeanor when interacting with the public and staff.
  • Strong social interaction, communication, and organizational skills with the ability to manage multiple tasks efficiently.
  • Valid driver's license and personal vehicle for travel.

Job Type: Full-time

Pay: $26.00 per hour

Expected Hours: 40 per week
