Patient Intake Specialist

Position Summary:
The Intake Coordinator is responsible for overseeing the entire patient intake process. This includes collaborating with the intake team, fostering positive relationships with clients and referral sources, addressing customer inquiries and concerns, and managing the verification and authorization of insurance.
Key Responsibilities:
- Assess hospitalized patients referred for home care services.
- Acquire a complete copy of the patient's medical history and physical examination.
- Verify insurance coverage for home health care services.
- Secure and document prior authorization for home care services from insurance providers.
- Assist in obtaining supplemental managed care orders under the supervision of the HMO Intake Manager.
- Negotiate payment rates for bundled home care services.
- Work closely with hospital discharge planning staff, utilization review departments, and insurance case managers to ensure safe and timely patient discharges.
- Consult with healthcare professionals to develop a coordinated home care plan.
- Ensure the completion of a Face to Face assessment.
- Conduct interviews with patients, families, and caregivers to discuss home situations, current needs, and relevant psychosocial factors.
- Complete referral documentation that includes intake data, essential background information, hospital course, and care plans.
- Maintain relationships with hospital and insurance personnel, providing information and education about services and coverage issues.
- Participate in patient care conferences and training sessions.
- Collect and maintain statistical data on all referrals and submit reports as required.
- Attend meetings and serve on committees as necessary.
- Notify the Director of Patient Care Services and Intake Supervisor of any delays in clinical care.
- Perform additional duties as assigned by the Administrator, Director of Patient Care Services, Director of Contract Management, and HMO Intake Supervisor.
Qualifications:
- High school diploma required; experience in secretarial and computer skills preferred.
- General knowledge of spelling, punctuation, grammar, medical records, and office procedures.
- Familiarity with office equipment.
- Basic telephone skills.
- Proficiency in data entry and current software systems.
- Ability to establish and maintain effective working relationships.
- Professional demeanor when interacting with the public and staff.
- Strong communication, organizational, and multitasking skills.
- Valid driver's license and personal vehicle for travel.
