Job Summary

We are seeking a highly skilled Clinical Coordinator to join our team at Tufts Medicine. As a Clinical Coordinator, you will play a critical role in the coordination of clinical services, ensuring the delivery of high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Triages calls related to patient care, including physician orders, staff issues, and other clinical issues.
  • Provides effective coordination of clinical services, consistent with the objectives of the agency.
  • Works closely with clinical staff to provide necessary services for patients and families in a timely manner, including the provision of direct patient care.
  • Communicates regularly with Clinical Managers and the Clinical Director on issues related to clinical practice or performance.
  • Assists with orientation and preceptorship of clinical staff.
  • Evaluates performance of each individual in relation to established standards in collaboration with the clinical manager.
  • Ensures compliance with regulatory guidelines and Tufts Medicine policies and procedures.
Requirements
  • Registered Nurse (RN) with three years of community health experience or a combination of at least one year of acute medical/surgical nursing experience and two years of home health experience.
