Job Summary

The Registered Nurse will play a vital role in our collaborative patient care team, providing direct patient care and demonstrating servant leadership. This role requires a strong commitment to delivering exceptional patient experiences, and the ability to work effectively in a team environment.

Key Responsibilities
  • Provide Evidence-Based Nursing Care: The Registered Nurse will provide high-quality, evidence-based nursing care to patients, including patient education and support.
  • Manage Patient Care: The Registered Nurse will manage patient care, connecting patients with specialists and following up to ensure patients receive the care they need.
  • Utilize Population Management Tools: The Registered Nurse will utilize population management tools to ensure patients receive optimal care based on industry-wide recommendations.
  • Provide Phlebotomy Services: The Registered Nurse will provide phlebotomy services, perform in-house laboratory tests, and work with the provider to determine how and when to send out specialty labs.
  • Screen for Vaccine-Preventable Diseases: The Registered Nurse will screen patients for vaccine-preventable diseases and administer primary care and travel-related immunizations.
  • Manage RN Schedule: The Registered Nurse will manage their own schedule and ensure appointments are appropriate. They will also take initiative to contact patients when needed and demonstrate knowledge of when to consult with a provider.
  • Comply with Policies and Procedures: The Registered Nurse will comply with all policies, procedures, and protocols of the practice, including performing specific care under standing orders.
  • Provide Triage Services: The Registered Nurse will provide general triage services to walk-ins and over the phone, assessing patient needs, consulting with the provider when needed, and directing appropriate care.
  • Participate in Quality Improvement: The Registered Nurse will participate in quality improvement programs to ensure the practice is performing efficiently and in accordance with Crossover Health Management Services Inc.'s policies.
  • Utilize Healthcare Analytic Tools: The Registered Nurse will demonstrate proficiency in the use of healthcare analytic tools, systems, and analysis.
  • Participate in Implementation of New Health Technologies: The Registered Nurse will participate in the implementation of new health technologies, products, services, and programs that establish Crossover Health Management Services Inc. as a recognized thought leader for employer-based healthcare and lifestyle medicine programs.
  • Manage Population Health Programs: The Registered Nurse will manage population health programs and clinic outreach initiatives.
  • Serve as a Resource: The Registered Nurse will serve as a resource to other nurses and the care team, and participate in the training and onboarding of new nurses.
  • Perform Quality and Compliance Checks: The Registered Nurse will assist in performing quality and compliance checks required on a daily, weekly, and monthly basis.
  • Contribute to Reporting Requirements: The Registered Nurse will contribute to reporting requirements related to incident reporting and compliance.
Required Qualifications
  • Graduate from an Accredited School of Nursing: The Registered Nurse must be a graduate from an accredited school of nursing.
  • Minimum 3 Years of Clinical Experience: The Registered Nurse must have a minimum of 3 years of clinical experience.
  • Current Registered Nurse License: The Registered Nurse must have a current Registered Nurse license in the state they are practicing.
  • Current BLS Certification: The Registered Nurse must have current BLS (Basic Life Support) certification.
Preferred Qualifications
  • Bachelor of Science Degree in Nursing: A Bachelor of Science Degree in Nursing is preferred.
  • Outpatient Care Experience: Experience in outpatient care and urgent care is preferred.
  • Knowledge of Healthcare Technology: Knowledge of Apple and Google-based technology or client technologies such as Microsoft or HP is preferred.
  • Passion for Delivering Exceptional Patient Care: A passion for delivering exceptional patient care in an integrated patient-centered primary care model is preferred.
  • Creative and Flexible: The Registered Nurse must be creative, flexible, and able to adapt to changing priorities.
  • Team Player: The Registered Nurse must be able to work effectively in a team environment and autonomously when needed.
  • Embraces New Ways of Delivering Healthcare: The Registered Nurse must be open to new ways of delivering healthcare.
Physical Job Requirements
  • Standing, Walking, and Sitting: The Registered Nurse will be required to stand, walk, and sit for extended periods of time.
