Job Overview

We are seeking a highly skilled Registered Nurse Case Manager to join our team based out of our Laconia, NH office. As a key member of our healthcare team, you will be responsible for managing a defined caseload of patients, providing direct patient care and comprehensive nursing services to individuals in the community.

About the Role

  • Valid nursing license in the state of NH is required
  • A minimum of 2 years of experience in skilled care is preferred
  • Experience in nursing supervision is also preferred
  • Recent wound care experience is essential
  • You must be able to lift 50 LBS with no restrictions
  • CPR certification is current
  • You will need reliable transportation

Estimated Salary

The estimated annual salary for this role is $79,200 - $87,960, based on a 38-43 hour workweek at $38-$43 per hour.
