We utilize a team-based model, including embedded Behavioral Health Clinicians, Case Managers, Care Coordinators, and Community Health Workers.

The Nurse Coordinator Role

The Team Nurse assumes responsibility for nurse visits at Amoskeag Health. They triage all telephone calls and walk-in patients with medical questions/concerns.

The position coordinates the flow of patient communications regarding labs, refills, dosages, changes, and instructions from providers.

Requirements

  • Qualifications: RN or LPN with 3 years experience. Current NH RN or LPN license.
  • Experience: Community health experience preferred.
  • Certifications: BLS Certification Required.
  • Skills: Ability to delegate tasks to other team members. Health assessment skills and coordination and delegation skills necessary.

Estimated Salary Range: $65,000 - $85,000 per year.
