Our Team

We utilize a team-based model that includes embedded Behavioral Health Clinicians, Case Managers, Care Coordinators, and Community Health Workers. This approach enables us to provide comprehensive, integrated, and affordable healthcare and education to our patients.

The Team Nurse Role

The Team Nurse assumes responsibility for nurse visits at Amoskeag Health. They triage all telephone calls and walk-in patients with medical questions or concerns. Additionally, this position coordinates the flow of patient communications regarding lab results, medication refills, dosages, changes, and instructions from providers.

Qualifications
  • Education: RN or LPN with 3 years of experience and current NH RN or LPN license
  • Experience: Community health experience preferred
  • Certification: BLS Certification Required
About the Position

We are seeking a skilled and compassionate Team Nurse to join our team. The estimated salary range for this position is $65,000 - $80,000 per year, based on location and experience.
