Role Summary:

The Permanent Housing Placement Liaison plays a crucial role in facilitating the transition of individuals and families from homelessness to permanent housing. This position is responsible for ensuring effective coordination of services across various departments, fostering smooth communication and collaboration among team members, community partners, and service providers. The Liaison will manage move-in activities, ensuring that participants experience a seamless transition into their new homes.

Key Responsibilities:

  • Manage participant transfers, ensuring timely processing and appropriate departmental coordination.
  • Maintain consistent communication with stakeholders, providing updates and facilitating warm handoffs.
  • Participate in weekly meetings with department leaders to discuss housing transitions.
  • Ensure adherence to move-in protocols, including lease collection and auditing.
  • Support accurate tracking of housing transitions and conduct regular audits for record accuracy.
  • Coordinate furniture orders, ensuring compliance with budgetary guidelines and processing purchase orders as necessary.
  • Oversee the complete transfer process, including tracking pending items related to housing transitions.
  • Input Move-In dates into the designated electronic database.
  • Facilitate and attend lease signings, ensuring all parties are present and informed.
  • Deliver necessary financial assistance checks during lease signings.
  • Process lease corrections and revisions as required.
  • Assist housing staff in identifying resources that support participants' housing stability.
  • Engage in ongoing training and professional development to stay informed about best practices in homelessness prevention and participant support.
  • Attend required meetings as directed by supervisors.
  • Utilize personal and agency vehicles for travel as needed.
  • Maintain a flexible work schedule, including availability for evenings and weekends.
  • Complete additional tasks and responsibilities as assigned by supervisors.

Qualifications:

  • Familiarity with tenant rights, lease agreements, and community resources.
  • Basic understanding of tenant/landlord laws and lease accuracy.
  • Strong interpersonal skills and cultural competence, with the ability to build rapport with diverse clientele.
  • Experience with electronic database systems, particularly HMIS.
  • Proficient in Excel for data management and analysis.
  • Strong advocacy skills, acting as a liaison to meet participant needs.
  • Creative problem-solving abilities focused on solutions.
  • Proficient in documentation and electronic data entry.
  • Excellent written and verbal communication skills.
  • Ability to work collaboratively and independently.
  • Bilingual skills in Spanish/English are preferred but not mandatory.
  • Knowledge of best practices such as the Housing First Model and Harm Reduction.

Additional Information:

  • Maintain confidentiality in accordance with HIPAA standards.
  • Obtain and maintain CPR/First Aid Certification.
  • Pass required health clearances.
  • Regular travel is expected for this role.
  • Possess a valid driver's license and maintain auto insurance.
  • Ability to work a 9/80 schedule.
