About the Role

This is an exciting opportunity to join LA Family Housing as a Patient Problem Solving Specialist, where you will play a critical role in coordinating efforts within the SPA 2 Coordinated Entry System (CES). As a liaison between hospitals and homeless service providers, you will foster relationships between organizations and facilitate transitions of care for individuals experiencing homelessness.

Key Responsibilities
  • Cultivate strong, collaborative relationships with hospital partners to ensure seamless coordination of care.
  • Train and educate hospital staff on access to resources within CES and other relevant resources for individuals experiencing homelessness.
  • Serve as a liaison between hospitals and community providers to ensure effective communication and problem-solving.
  • Participate in hospital case conferencing and collaborate in problem-solving efforts for individuals experiencing homelessness.
  • Provide problem-solving interventions to individuals experiencing homelessness and offer technical support to hospital staff as needed.
  • Assist in tracking participant's homeless service connection and re-connecting to providers as necessary.
  • Triage high acuity frequent hospital-utilizers and connect to CES staff as appropriate.
  • Refer patients experiencing homelessness to CES Access Sites for assessment and service connection.
  • Provide updates to homeless service providers who have participants entering hospitals.
  • Track metrics as assigned by supervisor and maintain accurate records for reporting purposes in HMIS database.
Requirements
  • At least two (2) years' experience in a system coordination role, preferably in a hospital setting, medical setting, or health care patient advocacy role.
  • Knowledge of homeless populations, systems, and programs, specifically the Coordinated Entry System (CES).
  • Ability to work independently in the field and as part of a team in various settings as necessary.
  • Ability to build relationships with stakeholders and present information thoroughly to a range of stakeholders.
  • Proficient in Microsoft Programs (Word, Excel, Access, PowerPoint), Zoom, and web-based software.
