Position Overview:
The Clinical Care Nurse for the LTSS Program will conduct comprehensive assessments, intakes, and care planning. Additionally, this role involves overseeing discharge plans for enrollees transitioning from hospitals or emergency departments, as well as monitoring notifications for care transitions support. The Clinical Care Nurse will assist the Program Director with daily operational tasks.

Key Responsibilities:
  • Facilitate team meetings and develop training materials as necessary for the program and Community Partners.
  • Engage in program evaluation and quality improvement initiatives related to the LTSS Community Partnership program and other associated activities.
  • Conduct comprehensive assessments, intakes, medication reviews, and care plans in accordance with program standards.

Qualifications:
  • Valid MA RN License
  • Minimum of three (3) years of clinical experience
  • At least three (3) years of experience in community nursing
  • Experience in care management within a health or human services organization
  • Prior experience working with individuals with disabilities
  • Ability to manage confidential and personal information appropriately
  • Proficient in computer applications
  • Strong written and verbal communication skills
  • Reliable transportation and a valid driver's license for service area visits

Hours: 40 hours per week, Monday through Friday.
