Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled and compassionate Home Health Physical Therapist to join our team in Amesbury, Massachusetts.

Job Summary:

We are looking for a highly motivated and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Physical Therapist, you will be responsible for assessing and treating clients with a variety of physical and cognitive impairments, and developing and implementing individualized care plans to help them achieve their goals.

Responsibilities:
  • Conduct thorough assessments of clients' physical and cognitive abilities to identify areas of strength and weakness.
  • Develop and implement individualized care plans to address clients' physical and cognitive needs.
  • Provide one-on-one therapy sessions to clients, including manual therapeutic exercises, gait training, balance, and other interventions.
  • Collaborate with other healthcare professionals, including nurses and occupational therapists, to ensure comprehensive care.
  • Document clients' progress and outcomes in a timely and accurate manner.
Requirements:
  • Current licensure as a Physical Therapist in the state of New Hampshire.
  • Minimum of one year of experience as a Physical Therapist in a home health setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic computer skills and familiarity with electronic medical records (EMR) systems.
