Responsibilities

As a Registered Nurse, you'll be responsible for performing visits, coordinating client care, and serving as the point of contact for all disciplines involved in patient care. You'll also oversee the frequency of visits, consult with physicians as needed, and ensure accurate OASIS collection and medication profiles.

Key responsibilities include attending weekly case conferences and monthly case manager meetings, as well as reporting lab values to physicians in a timely manner.

Qualifications

To be successful in this role, you'll need to be a currently licensed Registered Nurse in the state of employment, with a minimum of one year of clinical experience. You'll also need to demonstrate knowledge and skill in current nursing practice, possess a valid state driver's license, and have automobile liability insurance.
