As a Senior Community Care Advanced Practice Clinician (APC), you will play a vital role in providing comprehensive care to high-risk health plan members in a facility setting.

Salary: $40,000 Sign-on Bonus available for individuals who have not previously participated in this program or $30,000 Student Loan Repayment

Job Description
  • Conduct comprehensive assessments to identify members' needs and develop personalized care plans
  • Manage members through acute medical situations, exacerbations of chronic conditions, and short-term sub-acute rehabilitation episodes
  • Ensure accurate and complete documentation, including ICD-10 conditions
  • Collaborate with the interdisciplinary care team to coordinate care and achieve optimal outcomes
  • Provide advanced illness and end-of-life discussions, counseling, and education to patients and caregivers
Requirements
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • Active and unrestricted license in the state where you reside or ability to obtain by start date
  • National board certification as a Nurse Practitioner (e.g., American Academy of Nurse Practitioners or American Nurses Credentialing Center)
  • Current DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in your state)
  • Ability to gain a collaborative practice agreement, if applicable in your state
  • Access to reliable transportation
Preferred Qualifications
  • 1+ years of clinical experience in a long-term care setting
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Proficient computer skills, including documentation of medical information in electronic medical records
