Job Overview

This HouseCalls Clinician will conduct in-home assessments, perform screenings, and communicate findings to inform members' PCPs of potential gaps in care. You'll also educate members on disease processes, medication, and compliance.

The ideal candidate has experience as a Nurse Practitioner (NP) or Physician Assistant (PA), with strong clinical skills and excellent communication abilities.
