About the Role

We are seeking a skilled Advanced Care Specialist to join our Senior Community Care team at Optum. As an integral part of our interdisciplinary care team, you will provide comprehensive assessments, manage members with acute medical situations or exacerbations of chronic conditions, and ensure accurate documentation.

Key Responsibilities
  • Conduct thorough assessments to identify patients' needs and develop individualized care plans
  • Manage complex patient cases, including those with behavioral, social, and functional challenges
  • Collaborate with the care team to ensure seamless coordination of care and effective communication with patients and families
  • Provide education and counseling to patients and caregivers on disease management and self-care strategies
Requirements
  • Master's degree in Nursing from an accredited institution
  • Active and unrestricted license in the state where you reside or ability to obtain by start date
  • National certification as a Nurse Practitioner (e.g., AANP or ANCC)
  • Current DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
