Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at the Archdiocese of Philadelphia. As a Case Manager, you will be responsible for providing social work services to clients of our Residential Program, including intake, assessment, and case planning.

Key Responsibilities
  • Conduct thorough intake assessments of new clients to identify their needs and develop individualized service plans.
  • Develop and implement comprehensive case plans that address the client's physical, emotional, and social needs.
  • Provide counseling and support services to clients to help them achieve their goals and improve their overall well-being.
  • Collaborate with other service providers, such as medical and mental health professionals, to ensure that clients receive comprehensive care.
  • Monitor client progress and make adjustments to case plans as needed.
  • Document all client interactions and case plan progress in a timely and accurate manner.
  • Participate in regular team meetings and training sessions to stay up-to-date on best practices and industry developments.
Requirements
  • Bachelor's degree in Social Work or a related field.
  • At least two years of experience in social work or a related field.
  • Strong knowledge of homelessness, substance abuse, and mental illness.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
Preferred Qualifications
  • Master's degree in Social Work or a related field.
  • Experience working with homeless populations and providing case management services.
  • Knowledge of community resources and services available to homeless individuals.
Working Conditions

The Case Manager will work in a fast-paced environment and will be required to sit, stand, walk, and climb stairs as needed. The position may also require occasional travel to meet with clients and other service providers.
