Job Summary:
PeopleShare is seeking a skilled Case Manager to provide critical in-home services to at-risk families and individuals. As a key member of our team, you will work closely with clients to assess their needs, develop personalized service plans, and connect them with essential resources.
Key Responsibilities:
- Conduct Home Visits: Provide in-home services to at-risk families or individuals, including a hybrid caseload ranging from low to high risk referrals.
- Case Management Services: Conduct home visits, complete family assessments, create service plans, manage crisis intervention, advocate, connect to resources, and maintain accurate case files according to program standards.
- Collaboration and Communication: Coordinate with school personnel, DHS social workers, and service providers to communicate progress and work towards successfully connecting families to resources.
- Flexibility and Availability: Provide services and support outside of regular working hours to accommodate clients' needs when necessary. Be available on-call 24/7 for client emergencies (occasional).
- Professional Development: Regularly attend and actively participate in staff meetings, supervision, staff training sessions, case reviews, and other professional development opportunities.
Requirements:
- A minimum of 2 years of experience in a case management or related role.
- Previous experience navigating public benefits for clients.
- Experience working with school personnel.
- Bachelor's degree required.
- Mandated Reporter Certificate (within 5 years).
