Key Responsibilities:

As a Social Worker, your primary responsibilities will include:

  • Reviewing client case records to assess family profiles, socio-economic backgrounds, previous service experiences, and any special circumstances.
  • Developing comprehensive service plans that outline goals and objectives, determining the need for various social, behavioral, medical, and psychological services, and providing individual or family counseling as necessary.
  • Coordinating appropriate placements for individuals in kinship or family foster homes, including interviewing potential clients and providers, and ensuring understanding of program goals and legal implications.
  • Monitoring and evaluating the performance of contracted agencies providing social services, conducting site visits, and preparing written evaluations.
  • Initiating legal actions when necessary and preparing required documentation.
  • Completing a minimum of twenty (20) hours of training annually.
  • Participating in case conferences, school meetings, and interdisciplinary discussions to support case progress.
  • Providing family prevention and intervention services to enhance parenting standards and stabilize family dynamics.
  • Offering on-call services during emergencies on a rotational basis as per APM CUA protocols.

This is a full-time position with standard hours from Monday to Friday, 8:30 am to 5:00 pm, including an on-call component rotating every 4-5 weeks, with no weekend shifts.

All new hires are required to obtain necessary clearances, including FBI, Child Abuse Clearance, Criminal Background, Driving Record, and Medical/TB Test.

Qualifications:
