About the Role

As a Per Diem MSW Social Worker, you will play a vital role in our Case Management/Social Work department, collaborating with a multidisciplinary team to provide comprehensive care to patients and their families.

Key Responsibilities
  • Provide psycho-social support to patients and families, working closely with the unit/service CM/SW team to develop and implement discharge plans that ensure a safe and timely transition from hospital to home or community setting.
  • Collaborate with the behavioral health interdisciplinary healthcare team to ensure continuity of care and support services to patients and families.
  • Identify and address the psychological needs of patients and families, providing counseling and support as needed.
  • Document patient interactions, statistics, and reports in a timely and accurate manner.
  • Maintain relationships with community agencies and stay up-to-date on available resources.
  • Utilize effective interpersonal skills in all interactions, enhancing the overall patient experience.
Requirements
  • Licensed Social Worker preferred.
  • Master of Arts or Science in Social Work required.
  • 3+ years of behavioral health experience in a healthcare setting, with direct practice experience preferred.
