Position Overview


The Illinois Department of Public Health is seeking a highly motivated individual to serve as Field Operations Supervisor for Region 6.

This individual will oversee and manage all field operations within Region 6, located in Northern Illinois, including licensure, federal, and complaint surveys of long-term care facilities.

The Field Operations Supervisor is responsible for preparing and reviewing performance data, as well as developing strategies for performance improvement which may include managing staffing levels and training needs.

Job Responsibilities

  • Administers the implementation of the state licensure and federal certification survey process for long-term care facilities within Region 6.
  • Establishes long-term care survey schedules and ensures that survey activities comply with state and federal mandates.
  • Oversees all employees engaged in conducting State licensure surveys, federal certification surveys, and complaint surveys.
  • Travels to evaluate and supervise staff during on-site surveys.
  • Directs, coordinates, and reviews the long-term care activities of field supervisors, ensuring that policies and procedures are consistent across work units and aligned with the directives of the Division Chief and the Assistant Division Chief.
  • In collaboration with the Training and Technical Division, coordinates and implements all training and standardization activities for regional survey staff.
  • Travels to assess staff to determine individual training needs.
  • Implements uniformity in the application of survey policies and procedures.

Serves as full-line supervisor:

  • Assigns and reviews work.
  • Provides guidance and training to assigned staff.
  • Counsels staff regarding work performance.
  • Reassigns staff to meet day-to-day operational needs.
  • Establishes annual goals and objectives.
  • Approves time off.
  • Adjusts first-level grievances.
  • Effectively recommends and imposes discipline up to and including discharge.
  • Prepares and signs performance evaluations.
  • Determines and recommends staffing needs.
  • Serves as the Department liaison with the long-term care provider industry, the public, other state agencies, and federal officials regarding survey activities.
  • Develops and implements policies and procedures for field operations.
  • Assists the Division Chief and Assistant Division Chief in developing state-wide long-term care program policies and procedures.
  • Drafts recommended revisions to rules and regulations.
  • Develops survey documents, standard operating procedures, and forms in conjunction with the Training and Technical Division.
  • Meets and discusses the long-term care survey process with internal and external stakeholders.
  • Travels to attend conferences, meetings, and presentations.
  • Leads discussions on long-term care regulation in Illinois.
  • Provides technical assistance to external stakeholders on the survey process.
  • Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
Minimum Qualifications

Requires knowledge, skills, and mental development equivalent to completion of four years of college.
Requires prior experience equivalent to four years of progressively responsible administrative experience in Public Health related programs.

Preferred Qualifications

Four or more years of progressively responsible administrative experience in Medicare/Medicaid related programs.
Certified by the Centers for Medicare & Medicaid as having passed the Surveyor Minimum Qualifications Test (SMQT).
Four or more years of knowledge of state and federal regulations related to long-term care facilities.
Three or more years of supervisory experience.
Two years of experience establishing, maintaining, and improving cooperative working relationships with various stakeholders.
Two years of experience drafting, reviewing, and implementing policies and procedures.
Two years of experience with business communication with other agencies.
Two years of experience in Word, Excel, PowerPoint, and Smart sheets.

Conditions of Employment

Requires ability to travel in the performance of duties.
Requires appropriate, valid driver's license.
