Position Overview:

The Case Manager RN reviews patient records to assess clinical stability and actively participates in rounds to track patient progress. Additionally, they identify and address obstacles to care and work alongside social services to support patients' psychosocial requirements. Accurate data management, planning for medical discharge needs, and executing individualized action plans are essential responsibilities of this role. A valid Registered Nurse license and a minimum of two years of experience in Case Management are required.

Key Responsibilities:
  • Formulate and execute discharge strategies
  • Incorporate patient narratives into care plans
  • Facilitate access to services to enhance patient outcomes
  • Collaborate with various departments to remove care barriers
