Nursing Care Coordinator for Frail Elder Patients

Providence PACE is a Program of All-Inclusive Care for the Elderly that provides comprehensive care to older adults in need, allowing them to live active, independent lives.

We are seeking a per diem/on call Registered Nurse to support our Beaverton site and surrounding areas as needed, providing care management, care coordination, skilled care and services for at-risk frail elder patients with complex medical and psychosocial issues.

This role requires travel between multiple locations and involves working closely with the patient centered PACE interdisciplinary team (IDT) to ensure a systematic approach to the coordination of health services for patients along the care continuum.

The successful candidate will have strong communication and organizational skills, with the ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.

This position is responsible for planning, overseeing and monitoring all nursing and unlicensed caregiver services to each patient on their panel within the context of an IDT.

The goals of care management are to assure a systematic approach to the coordination of health services for patients along the care continuum in order to improve the patient experience, contain health expenditures and improve quality.

This position requires Oregon Registered Nurse License upon hire and National Provider BLS - American Heart Association certification within 30 days of hire.
