Primary Care Nurse

In this role, you will provide comprehensive nursing care for patients in an ambulatory care environment based on individual physical, emotional, and spiritual needs. You will facilitate care coordination by using the nursing process to assess, identify, plan, implement, and evaluate all aspects of care and teaching needs via direct and telephonic/telehealth or electronic encounters.

Key Responsibilities- Provide comprehensive nursing care for patients in an ambulatory care environment
- Facilitate care coordination by using the nursing process
- Assess, identify, plan, implement, and evaluate all aspects of care and teaching needs
- Graduate of an accredited registered nursing school
- Associate's Degree in Nursing or Bachelor's Degree in Nursing
- Oregon Registered Nurse License
- National Provider BLS - American Heart Association
- 18 months professional nursing experience
