Job Responsibilities
As an RN in our PACE program, your responsibilities will include:
  1. Providing care management, care coordination, skilled care, and services to at-risk frail elder patients.
  2. Collaborating with our interdisciplinary team to develop and implement comprehensive care plans.
  3. Assisting providers in identifying patient needs and coordinating access to appropriate care and services.
  4. Monitoring patient outcomes and adjusting care plans as needed.
Our RNs are responsible for the planning, overseeing, and monitoring of all nursing and unlicensed caregiver services to each patient on their panel within the context of an IDT. This position requires travel between multiple locations.

Requirements
To be successful in this role, you will need:
  1. A current Oregon Registered Nurse License.
  2. National Provider BLS - American Heart Association certification within 30 days of hire.
  3. Relevant certifications applicable to the position, as specified by the Nurse Manager.
We prefer candidates with a Bachelor's Degree in Nursing and 1 year of home health RN experience. However, we encourage applicants from all backgrounds to apply.
