About the Position

This Per Diem/On Call RN position requires a unique blend of clinical expertise, communication skills, and compassion. You will work collaboratively with our interdisciplinary team to provide comprehensive care services to frail elderly patients, promoting their independence and quality of life.

Responsibilities:

  • Assess patients' physical, emotional, and social needs, developing individualized care plans that address these complexities.
  • Implement and evaluate care plans, ensuring continuity and coordination across all aspects of patient care.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions and optimal outcomes.

Benefits:

  • $40.20 - $63.64 per hour base pay range, with additional compensation for shift differentials, standby/on-call, overtime, premiums, and bonus opportunities.
