About the Job

We are seeking a skilled Registered Nurse (RN) to join our team at Providence Elderplace in Beaverton, OR.

The ideal candidate will possess excellent nursing skills and have experience in providing care management, care coordination, and skilled care services to at-risk frail elder patients with complex medical and psychosocial issues.

This is a full-time day shift position that requires travel between multiple locations. The successful candidate will be responsible for planning, overseeing, and monitoring all nursing and unlicensed caregiver services to each patient on their panel within the context of an interdisciplinary team (IDT).

About the Role

This position is integral to the PACE interdisciplinary team and plays a critical role in ensuring the systematic approach to the coordination of health services for patients along the care continuum.

The goals of care management are to improve the patient experience, contain health expenditures, and improve quality. This position assists providers in effectively identifying needs and coordinating access to appropriate care and services in an effort to improve or maintain the patient's functional status and independence.

The ideal candidate will have strong communication and organizational skills, with the ability to work collaboratively with healthcare professionals and community resources to achieve optimal patient outcomes.
