Frail Elder Patient Care Specialist

The Providence Home and Community Care (HCC) team is seeking a skilled Frail Elder Patient Care Specialist to join our program. As a member of our interdisciplinary team, you will provide care management, care coordination, and skilled care services for at-risk frail elder patients with complex medical and psychosocial issues. This role requires travel between multiple locations and strong communication and interpersonal skills.
Key Responsibilities:
• Provide patient-centered care management, including needs assessment, goal setting, and service coordination
• Work closely with healthcare providers, family members, and community resources to ensure comprehensive care
• Develop and implement individualized care plans that address the unique needs of each patient
• Monitor patient progress, make adjustments to care plans as needed, and communicate effectively with stakeholders
• Participate in regular team meetings to discuss patient cases and share best practices
Requirements:
• Bachelor's Degree in Nursing (BSN)
• At least 1 year of experience as a Home Health RN
• Strong communication and interpersonal skills
• Ability to work independently and collaboratively as part of a team
• Proficiency in electronic medical records (EMRs) and other software applications used by Providence HCC
