About the Role

This per diem/on call position supports our Beaverton site and surrounding areas as needed. You will provide care management, coordination, skilled care, and services for frail elder patients with complex medical and psychosocial issues.

Key Responsibilities
  • Coordinate patient care within the interdisciplinary team to achieve optimal outcomes
  • Provide comprehensive assessments, care planning, and implementation of individualized care plans
  • Collaborate with healthcare providers to ensure seamless transitions and continuity of care
  • Develop and maintain relationships with community resources to support patients' needs
  • Travel between multiple locations to provide care and services as needed
Requirements

To be successful in this role, you will need:

  • Oregon Registered Nurse License upon hire
  • National Provider BLS certification within 30 days of hire
  • Relevant certifications applicable to the position as specified by the Nurse Manager
  • Bachelor's Degree in Nursing (preferred)
  • 1 year Home health RN experience (preferred)
