Care Coordinator Job Description

The Care Coordinator is a highly visible role that requires strong customer service and patient-focused skills. You'll work directly with patients, families, insurance representatives, and healthcare professionals to ensure seamless referrals and high-quality care.

Key Responsibilities:
  • Coordinate and process patient referrals with precision, detail, and accuracy.
  • Prioritize patient referrals, expedite orders, and schedule appointments with specialists.
  • Use Web IVR to generate authorizations and complete orders with proper documentation.
  • Participate in daily huddles, communicate with physicians and clinicians, and ensure patient needs are met.
  • Enter patient information into HITS tool, reschedule missed appointments, and provide exceptional customer service.
  • Follow up on Home Health and DME orders, and perform other related duties as assigned.
Requirements:
  • High School diploma or equivalent required.
  • Minimum 1 year of relevant work experience in customer service, patient referrals, and/or insurance verification.
  • Experience with Web IVRs and obtaining referrals/authorizations for multiple payers strongly preferred.
  • Healthcare experience in a clinical setting, preferably within the Medicare HMO population, is a plus.
