Key Responsibilities:
The MDS Nurse, also known as the Care Management Nurse, is responsible for overseeing the RAI process and conducting thorough assessments and care plan coordination for assigned residents. Key duties include:
- Completing the MDS, CAAs, and care plans within required timelines.
- Performing resident assessments through physical examinations, interviews, and chart reviews.
- Collaborating with caregivers, including physicians, nursing staff, social services, therapy, dietary, and activities personnel, to discuss resident care needs.
- Reviewing information from hospitals and external agencies to inform assessment and care planning.
- Coordinating and identifying significant changes in MDS.
- Preparing for and conducting PPS meetings, ensuring MDS assessments align with Medicare schedules, and maintaining the PPS board for monitoring Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
- Staying updated with the requirements set forth by the American Association of Nursing Assessment Coordinators (AANAC).
Qualifications:
- Must be a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
- AANC certification is preferred, with RAC-CT being a plus.
- Strong understanding of the Resident Assessment Instrument (RAI) process and the Prospective Payment System (PPS) is highly desirable.
- Previous experience as an MDS Nurse is essential.
