The GW Medical Faculty Associates is seeking a skilled Clinical Nurse Navigator/Coordinator to join our Internal Medicine team. As a clinical nursing expert, you will play a vital role in providing exceptional patient care, serving as a valuable resource, advocate, and educator.
Key Responsibilities:
- Deliver patient-centered care in a collaborative environment, prioritizing individual needs and preferences.
- Ensure seamless communication with patients regarding test results, procedures, and follow-up appointments.
- Prepare comprehensive clinic notes for new and returning patients, guaranteeing accurate and up-to-date information.
- Assist in coordinating follow-up visits, orders, and referrals, promoting efficient care transitions.
- Contribute to quality improvement initiatives, identifying areas for enhancement and implementing evidence-based practices.
- Participate in training and educating support staff, fostering a culture of excellence and continuous learning.
Requirements and Qualifications:
Education: Associate's degree or higher in Nursing, or equivalent training and experience.
Licensure: Current and valid D.C. RN license or evidence of application for licensure.
Experience: Minimum of 2 years as a Registered Nurse, with a strong background in internal medicine.
Physical Requirements: Ability to move comfortably in a clinical setting, lift up to 100 lbs., and use necessary protective equipment.
