Job Summary

The Clinical Nurse Navigator/Coordinator is a vital member of our multidisciplinary team, providing clinical nursing expertise in an assigned area. This role focuses on the clinical and educational aspects of care, acting as a clinical resource, advocate, and educator.

Key Responsibilities
  • Demonstrate a strong commitment to patient-centered care within an interdisciplinary environment.
  • Follow up with patients on testing and procedure results in coordination with providers.
  • Prepares Clinic prep notes for new patients and returning patients, including scanning notes/imaging, contacting referring physicians or imaging groups, and contacting patients on any issues or items needed for visit.
  • Assists with coordination of follow-up visits, orders, and referrals.
  • Initiates/Participates in quality improvement, patient safety, and patient experience initiatives.
  • Participates in training and education of support staff.
Requirements
  • An Associate's degree or higher in Nursing, or an equivalent combination of training and experience.
  • A current and valid D.C. license as a Registered Nurse or evidence of application for licensure in D.C.
  • A minimum of 2 years of experience as a Registered Nurse.
