Clinical Nurse Navigator/Coordinator, Neurology

MFA Co is a leading healthcare provider in the DC metro region, offering comprehensive patient care with 52 medical and surgical specialties. Our physicians are members of the GW School of Medicine and Health Sciences faculty, providing academic, research, and healing services.
We have a closely integrated alliance with The George Washington University and The George Washington University Hospital (GWUH), which is separately owned and operated by Universal Health Services (UHS). Our active referring relationships with 12 area hospitals enable us to provide seamless care to our patients.
Our healthcare presence in the DC metro region is complemented by a network of community-based practices in DC, Maryland, and Virginia. Given our geographic location in central NW Washington, DC, and proximity to more than 175 resident embassies, we continue to evolve our international clinical outreach.
Job SummaryThe Clinical Nurse Navigator/Coordinator is a vital member of our multidisciplinary team, providing clinical nursing expertise in an assigned area. This role focuses on the clinical and educational aspects of care, acting as a clinical resource, advocate, and educator.
Key Responsibilities- Demonstrate a strong commitment to patient-centered care within an interdisciplinary environment.
- Follow up with patients on testing and procedure results in coordination with providers.
- Prepares Clinic prep notes for new patients and returning patients, including scanning notes/imaging, contacting referring physicians or imaging groups, and contacting patients on any issues or items needed for visit.
- Assists with coordination of follow-up visits, orders, and referrals.
- Initiates/Participates in quality improvement, patient safety, and patient experience initiatives.
- Participates in training and education of support staff.
- An Associate's degree or higher in Nursing, or an equivalent combination of training and experience.
- A current and valid D.C. license as a Registered Nurse or evidence of application for licensure in D.C.
- A minimum of 2 years of experience as a Registered Nurse.
