Nurse Navigator

The Nurse Navigator - Family Medicine Practice is a key role in our team, responsible for coordinating patient care across the continuum. This position requires a strong understanding of nursing practice, national guidelines, and the ability to educate and support patients in understanding their treatment plans.
Key Responsibilities- Assessment: Obtain a baseline of information to develop an individualized plan of care.
- Planning: Work with an interdisciplinary team to develop and manage the plan of care for patients.
- Patient Education: Provide initial and continuing education related to specific disease processes and treatment modalities.
- Implementation: Act as a liaison between patients, families, and healthcare teams to coordinate care.
- Evaluation: Monitor patient progress and evaluate the effectiveness of care.
- Associate's degree in Nursing required; Bachelor's degree in Nursing preferred.
- 3-4 years of progressively more responsible clinical nursing experience in specialty area required.
- RN - Registered Nurse - State Licensure and/or Compact State Licensure in the District of Columbia required.
- CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required.
