Partners For Quality is a nonprofit organization dedicated to providing community-based support for individuals facing behavioral health challenges and those with intellectual and developmental disabilities across Western Pennsylvania.

Responsibilities:

PRIMARY FUNCTIONS:

This role is centered on empowering individuals who are navigating physical or intellectual challenges, those on a behavioral health journey, or members of the Deaf community to reach their fullest potential.

Our organization offers a variety of services aimed at enhancing overall health outcomes by addressing key aspects of Holistic Health that extend beyond conventional healthcare, focusing on:

1. Person-centered activities that encourage safe and sustainable independent living
2. Opportunities to enhance community involvement and social support for those we serve
3. Educational initiatives and promotion of health literacy
4. Ensuring access to nutritious food and dietary resources
5. Improving economic stability through exploration of employment-related activities


This diverse role requires collaboration with Behavioral Health Therapists to design therapeutic programs, implement and co-facilitate individual or group activities both within the facility and in the community, and assess outcomes to meet established objectives.

Your support and tailored interventions will be crucial in assisting individuals to overcome obstacles and develop vital life skills.


ESSENTIAL FUNCTIONS:
1. Collaborate with the Behavioral Health Therapy team and individuals served to set realistic, measurable, and achievable treatment goals
2. Regularly review and adjust treatment goals and plans based on client progress, evolving needs, or new circumstances
3. Coordinate and manage all therapeutic activities, including scheduling, securing necessary supplies and transportation, budgeting, and resource allocation
4. Co-facilitate engaging activities or discussions that foster personal growth, self-awareness, and skill development, respecting the diverse perspectives within the group to meet individualized objectives
5. Provide activity opportunities that foster skill development and identify community activities for participation
6. Implement applicable behavioral support plans and utilize positive strategies to assist individuals receiving services
7. Enforce individualized crisis plans, as necessary, in accordance with regulations and guidelines
8. Encourage and assist individuals in participating in activities, providing support and motivation as needed
9. Consult with the Social Rehabilitation Director and other external service providers to ensure comprehensive care
10. Maintain a safe and welcoming environment for all individuals served
11. Complete accurate and professional documentation in a timely manner, including assessments, progress notes, census, billing records, and other relevant program documentation, adhering to established organizational policies and contractual requirements.
12. Comply with all organizational policies and procedures and complete additional duties as assigned.

QUALIFICATIONS:
Associate degree in human services, social sciences or related field and 2 years of field experience; OR
Bachelor's degree in human services or related field and 1 year of field experience
Experience coordinating activities for individuals with diverse abilities, including those navigating physical or intellectual differences, on a behavioral health journey, or members of the Deaf community
Act 33 and Act 34 clearance and FBI background check, if applicable
Valid PA Driver's License
