Partners For Quality is a nonprofit organization dedicated to supporting individuals facing behavioral health challenges and those with intellectual and developmental disabilities across Western Pennsylvania. We are recognized for our commitment to trauma-informed care.


Key Responsibilities:

PRIMARY FUNCTIONS:

The primary objective of this position is to empower individuals with diverse abilities, including those on a behavioral health journey and members of the Deaf community, to reach their fullest potential.

Our organization provides a range of services aimed at enhancing overall health outcomes by focusing on holistic health aspects that extend beyond conventional healthcare. Key areas of focus include:

  • Facilitating person-centered activities that encourage safe and sustainable independent living.
  • Creating opportunities for community engagement and social support.
  • Promoting educational initiatives and health literacy.
  • Ensuring access to nutritious food and healthy eating.
  • Enhancing economic stability through employment-related activities.

This diverse role involves collaboration with Behavioral Health Therapists to design therapeutic programs, implement and co-facilitate individual or group activities both within the facility and in the community, and assess outcomes to meet set objectives.

By offering tailored support and interventions, you will play a crucial role in assisting individuals in overcoming challenges and developing essential life skills.


ESSENTIAL FUNCTIONS:
  • Work collaboratively with the Behavioral Health Therapy team and individuals served to set realistic, measurable treatment goals.
  • Regularly assess and update treatment plans based on client progress and evolving needs.
  • Coordinate and manage all therapeutic activities, including scheduling, securing supplies, budgeting, and resource allocation.
  • Co-facilitate engaging activities that foster personal growth and skill development, respecting the diverse experiences within the group.
  • Provide opportunities for skill development and identify community activities for participation.
  • Implement behavioral support plans and utilize positive strategies to assist individuals receiving services.
  • Maintain a safe and welcoming environment for all participants.
  • Complete accurate and professional documentation in a timely manner, including assessments, progress notes, and other required records.
  • Adhere to all organizational policies and procedures.
  • Perform additional duties as assigned.

QUALIFICATIONS:
  • Associate degree in human services, social sciences, or a related field with 2 years of relevant experience; OR
  • Bachelor's degree in human services or a related field with 1 year of relevant experience.
  • Experience in coordinating activities for individuals with diverse abilities, including those on a behavioral health journey or members of the Deaf community.
  • Clearances as required by regulations.
  • Valid PA Driver's License.
