Responsibilities:

JOB SUMMARY


This position is responsible for gathering precise and current information regarding patients' home medications. This includes documenting prescribed and over-the-counter medications taken by patients and entering this data into the electronic medical record system according to established protocols.

Employees in this role will generate an admission medication reconciliation document for pharmacist review and ensure it is available for physician assessment as necessary.

This role requires effective communication skills to engage with patients and their families, often in high-pressure situations.

Collaboration with the pharmacy department and nursing staff is essential to relay critical information that supports patient care, as well as liaising with physician offices and retail pharmacies to verify medication details for accuracy.

Key responsibilities include enhancing the medication reconciliation process to ensure compliance with regulatory standards concerning the collection and verification of home medication information.


ESSENTIAL JOB FUNCTIONS:


Conduct interviews to collect comprehensive information about all current home medications from patients or caregivers, primarily in the Emergency Department. This involves asking relevant questions regarding dosages to clarify information and facilitate timely and accurate data collection.


Document the information obtained during interviews into the electronic medical record system in accordance with established procedures to create the necessary admission medication reconciliation documentation.

Generate documentation for review by pharmacists and subsequent physician evaluation.


Assist other healthcare professionals in the Emergency Department and other areas as needed to ensure timely provision of medications, collaborating with inpatient pharmacy staff as required.


Cultural Sensitivity and Competence:
Demonstrate effective communication techniques and an understanding of how cultural factors influence patient interactions and the delivery of care.

Patient Population Served:
Exhibit knowledge and skills pertinent to the specific patient populations served by the department.

Perform related duties as assigned.

Qualifications:
Education/Work Experience Requirements

Completion of an accredited Pharmacy Technician program, along with one of the following: 1) at least one year of pharmacy technician experience in an inpatient or outpatient setting, or 2) one year of experience in a customer service role involving direct interaction with clients that demonstrates the necessary job knowledge and skills.

Licensure/Certification

Must be licensed as a Pharmacy Technician by the appropriate state board of pharmacy.

Current Basic Life Support (BLS) certification for healthcare providers.
