Nurse Practitioner

CharterCARE Health Partners is seeking a skilled Registered Nurse to provide high-quality nursing services in the home environment. As a key member of our team, you will work closely with our patients to develop and implement personalized care plans that meet their unique needs.
The ideal candidate will have a strong background in medical surgical nursing and experience working in a home health care setting. You will be responsible for providing skilled nursing services, including wound care, medication management, and education to patients and their families.

Responsibilities:
- Provide skilled nursing services in the home environment
- Develop and implement personalized care plans for patients
- Collaborate with patients, families, and healthcare teams to ensure high-quality care
- Manage medication and treatment plans
- Provide education and support to patients and their families
Requirements:
- Current RN licensure in the States of Rhode Island and Massachusetts
- One to two years of medical surgical nursing experience in an acute care facility
- Experience working in a home health care setting
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
