About the Role:This is a unique opportunity to work in a dynamic and innovative healthcare environment, where you will have the chance to make a meaningful impact on the lives of our patients.
Key Responsibilities:- Provide high-quality patient care as a Nurse Practitioner, with a focus on primary care, internal medicine, geriatrics, and family medicine.
- Develop and implement population health strategies, working closely with the Care Team to ensure evidence-based care and optimal patient outcomes.
- Conduct routine and acute office visits, as well as home visits as needed, to provide comprehensive care to our patients.
- Administer annual wellness visits and health risk assessments, requiring a holistic view of health and accurate documentation.
- Collaborate with the Care Team to ensure all eligible females receive evidence-based breast cancer screening every 2 years.
- Competitive salary and quarterly bonus based on quality metrics.
- 6 weeks of PTO, inclusive of vacation, sick time, major holidays, and continuing medical education (CME).
- $5000 CME stipend.

- Active DEA.
- Experience in primary care: internal medicine, geriatrics, or family medicine.
- Understanding of HCC (Hierarchical Condition Categories) documentation, ICD10 (International Classification of Diseases10) Coding, and Health Risk Assessments (HRAs).
