Job Summary

The Population Health Care Manager is a critical role in our organization, responsible for delivering clinical expertise to manage the health care needs of specific patient populations across the continuum of care. This position focuses on improving the health status and connection to resources, preventive care, hospital follow-up, and ongoing healthcare for individuals with chronic health conditions.

Key Responsibilities
  • Develop and implement care plans to address the medical, behavioral, and psychosocial needs of patients with chronic health conditions.
  • Collaborate with interdisciplinary teams to optimize clinical outcomes and reduce unnecessary utilization and cost.
  • Provide education and support to patients and families to promote self-management and health behavior change.
  • Monitor and evaluate the effectiveness of interventions and make adjustments as needed.
  • Document all care management activities in Maestro and other relevant systems.
Requirements
  • Bachelor's degree in Nursing or Master's degree in a related field.
  • 3 years of relevant clinical experience.
  • Current licensure as a Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) in the state of North Carolina.
  • Case management certification (ACM, CCM, or ANCC) within 3 years of hire.
Preferred Qualifications
  • Experience in population health management and care coordination.
  • Knowledge of disease management and chronic disease support.
  • Excellent communication and facilitation skills.
